A. With Appointment: Please let our Patient Care Specialist know which doctor you requested to see upon entry and for what time.

B. Without Appointment

    • First-time visitors are asked to complete a New Patient Form and present a photo ID.
    • Patients who have not visited for more than 3 years are asked to complete an Updated contact information form (if required).

Additional documents that may be requested:

    • Medical reports/lab tests to add to your file for the Doctor
    • Any Insurance forms
